Job Description

The Organisation

The SSS religious community reside in Melbourne’s CBD and provide sacramental ministry at St Francis Church Lonsdale St Melbourne.  There are also some religious residing at an Aged Care Facility in Northcote.

The St Francis Community are seeking to appoint a part time Health and Wellbeing Coordinator to assist with the care of their religious.

 

The Role

Working in close consultation with the Congregational Community Leader, the Health and Wellbeing Coordinator is responsible for identifying individual and community needs and planning and coordinating formal services and informal support.

Where required this also includes individual care delivery to enable the highest quality of life for the members of the community. The position includes collaborating with community nominated health and wellbeing peer supports.

Responsibilities of the position include but are not limited to:

  • Acting as the point of contact for community members in respect of their health and wellbeing support needs
  • Supporting Religious’ independence and continued participation within the community
  • Providing advocacy to promote the health and wellbeing of the individual
  • Supporting individuals and the community to improve health and aged care literacy;
  • Building trusted, therapeutic relationships with the Community Leader, health and wellbeing peer supports, Community Residence Manager, and community members.

 

The Requirements

  • Nursing or allied health professional qualifications in aged care and community services
  • Current AHPRA registration (if applicable)
  • Well-developed interpersonal and leadership skills
  • Proven relationship management skills and experience with internal and external stakeholders
  • Current Level 2 First Aid Certificate
  • Current Australian driver’s license
  • Current Police Record Check

 

Hours of Work and Remuneration

This ongoing part time position will be for:

  • Monday 9:30am-1:30pm
  • Thursday 9:30am-1:30pm

with some flexibility to work extra hours as required and agreed.

The salary will be commensurate with experience and qualifications.
