Job Description

The Role

The Health and Wellbeing Coordinator acts as a trusted and knowledgeable point of contact for the Community Leader, community members and health and wellbeing team members.  Reporting to the Health and Wellbeing Manager the coordinator will identify individual and community needs for the Sisters, plan and coordinate formal services, review resource allocation and advocate for the wellbeing of Sisters.

The Health and Wellbeing Coordinator will support and guide the health and wellbeing team members to ensure care is person centred to promote an individual’s strengths, independence, and autonomy.

The Requirements

Ideally will have experience working in a similar role and have a practical understanding of home care packages and other aged care services, with key responsibilities including (but not limited to):

  • Creating and maintaining a trusted relationship with the Community Leader and Health and Wellbeing Manager
  • Supporting and encouraging the assessment of sisters eligible for Home Care Packages
  • Providing a full oversight of all care provided to Sisters in the community
  • Coordinating and monitoring effective and efficient access and coordination of care and services for the Sisters across the community
  • Assisting the Sisters to engage lifestyle choices which promote healthy ageing, person centred care and independence
  • Liaising with formal service providers and make/manage referrals as required
  • Building, maintaining and leading an effective and harmonious team
  • Building and developing the skills, capabilities and facilitating education for the health and wellbeing team, in consultation with the Health and Wellbeing Manager
  • Maintaining effective reporting that is accurate, timely and in accordance with the organisation’s requirements
  • Reviewing documentation of team members and monitoring their schedules
  • Maintaining staff and Sisters’ confidentiality at all times

In addition, you will liaise on a regular basis with the Community Leader with open and respectful communications regarding the long-term care of Sisters and report any concerns or significant changes regarding a Sister’s health and wellbeing.

Ideally you will have worked in an environment which has supported older people to manage their health and wellbeing needs as they age, whether this be in a community or residential/medical setting.

In-depth knowledge of home care packages and care coordination is highly valued. Your natural empathetic, caring and respectful manner along with your ability to take a holistic view on a person’s health & wellbeing, and a strong ability to take initiative and show sound judgement will be well rewarded in this role.

This is a 2 years maximum term part time position working approx. 10 – 15 hrs per week (flexible to fit in with your lifestyle).  The position is based in the greater Perth metropolitan area and is being paid an hourly rate commensurate with the requirements of the role.  Please note, you will need to have a current driver’s license to travel to residential and aged care homes.
