Job Description

We are seeking a compassionate, organised and proactive Wellbeing and Health Coordinator to oversee and lead the care of sisters living in Geelong, Ballarat, Warrnambool, Apollo Bay, Bendigo and surrounding areas.

The Role:

The Wellbeing and Health Coordinator is a trusted and knowledgeable point of contact to the Community Leadership Group, Sisters within the Community and the wellbeing and health team members.

Reporting to the Wellbeing and Health Manager, the Coordinator will identify individual and community needs for the Sisters across the community. In liaison with Sisters and team members you will provide oversight of the care for Sisters, review resource allocation and advocate for the wellbeing of Sisters.

The Wellbeing and Health Coordinator will lead and guide the wellbeing and health team members to ensure holistic and person-centred care which promotes an individual’s strengths, independence and autonomy.

The Requirements:

Ideally you will have experience working in a similar role and have:

  • Demonstrated leadership experience which includes leading a team
  • Experience in care coordination and knowledge of Government subsidised services available to older Australians
  • Strong communication and interpersonal skills with the ability to build trusting relationships
  • Proficient organisational and problem-solving abilities

Your natural empathetic, caring and respectful manner is key. Your ability to take a holistic view on a person’s wellbeing and health, navigate and manage change and take initiative will be well rewarded in this role.

Key responsibilities including (but not limited to):

  • Oversight of care for the Community members, which includes monitoring and coordinating effective and efficient access of care and services
  • Provision of advocacy to support a Sisters transition across life stages
  • Promotion of lifestyle choices which support healthy ageing and independence
  • Liaison with formal services and health providers to make/manage referrals as required
  • Build and develop the skills and capabilities of the wellbeing and health team and facilitate education/training
  • Maintain reporting to ensure it is effective, accurate, timely and in accordance with the organisation’s requirements
  • Oversee team members’ documentation and coordinate rosters/time management
  • Maintain Sisters’ confidentiality at all times

Ideally you will have worked in the community or residential care sector. Experience in health-related field or social work field is highly valued.

This is a 2 Year Maximum Term Contract, full-time position working 37.5 hrs per week Monday to Friday. The hourly rate is commensurate with the requirements of the role.

Please note, you will need to have a current driver’s licence and be willing to travel across the Geelong, Ballarat, Warrnambool, Apollo Bay, Bendigo and surrounding areas. An Institute car will be provided for work related travel. You will enjoy a variety of work that includes both office work and connection with Sisters and staff.
