The Role

The position will provide support to Sisters of Mercy residing in the Perth and surrounding areas. Most sisters reside in the community setting with a number living in residential aged care facilities. The primary purpose in the community is to monitor and plan for future wellbeing and health needs, assist with referral and assessment for Government subsidised services which support a person to receive care in their home, assist with coordination of care, develop engagement with wellbeing activities and promote healthy ageing.
The Requirements
You will have experience working in a caring role and the understanding of the physical and social needs of others, as well as the ability to support individuals to maintain their independence and plan for future needs.
Your natural empathetic, caring and respectful manner is key. Your ability to take a holistic view on a person’s wellbeing and health, navigate and manage change and take initiative will be well rewarded in this role.
Skills and experience:
- Background in health, caring for others, community care, social sciences, wellbeing and
- Experience in care coordination and knowledge of Government subsidised services available to older Australians
- Strong communication and interpersonal skills with the ability to build trusting relationships
- A strong desire to work with the elderly and promote independence
- Experience in care coordination (desirable)
- Able to take a holistic view of a person’s wellbeing & health
- Competent written and verbal communications skills and good computer skills.
This is a 12 month maximum term part time position (with potential for extension) working 20 hours per week (across 3-4 days per week), with the potential for additional hours. Flexibility in work hours and days may be required and would be highly regarded.
You will be paid an hourly rate commensurate with the requirements of the role. Please note, you will need to have a current driver’s licence and have the ability to travel to areas throughout Perth. Travel will be required on a needs basis and in consultation with the Wellbeing & Health Coordinator (or her delegate).   A car can be provided for travel.
